The app.js is empty. The index.xml can be a blank as well. This should be enough to re-create the app. Also edited the question. Note that I am using the Alloy framework. Please inform me if you need anything else.

I've attached the ZIP file to the Jira issue. Note that I couldn't include the module myself, as there is a 10mb limit. However, you can easily extract the module into the appropriate location. My question also has more detail.
