[TIMOB-11101] Win8: "Category" and "Department" fields aren't cleared after adding a charge

Description
When user fills "Add charge" fields and clicks "Add to Report", text fields are cleared, but the picker fields keep the values that were selected.
For simplicity sake, add an empty row to the picker and set it through the JS code when the charge is added to the report table. Make sure entry validation is adjusted accordingly.
Not clearing date field when adding charge. It's unlikely to change so it speeds up data entry. No need to clear picker boxes.
